Health & Medical Forms
The school must keep on file a health report signed by a doctor listing immunizations and illnesses. This report must be given to the school upon enrollment and will stay in the child’s cumulative record. Schools in Michigan are required to assess and report the immunization status of all students. Michigan requirements are as follows:
- 2 doses of MMR
- 3 doses of Hepatitis B
- complete series of DTaP/Td with 1 dose in the last 10 years
- 3 doses of Polio
- 2 doses of Varicella (of a history of chicken pox disease)
Families opting out of immunizations must sign a Contraindication or waiver form to formalize their exemption. Waivers can be obtained from a physician or BCHD (Berrien County Health Department). Official Immunization reports must be presented at enrollment – for students new to ROL.
Dress Code & Uniforms

By conduct and appearance, our students should glorify God at all times. In keeping with these goals and standards, we encourage positive values, speech, modesty, cleanliness, and safety. We also desire to promote an atmosphere that is conducive to learning, encourages responsible decision making and dress that does not distract from our learning community.

Pickup Procedures
If you need to pick up your student early, please notify the ROL office (269-757-7549) as soon as possible. Please do not go directly to your student’s classroom. Sign your child out in the office and a staff member will get your child from their classroom to avoid disrupting learning. If your child will have different pick-up arrangements other than the parent/guardian indicated on file in the office, please alert the office for your child’s safety.
